- coronary embolism is possible by 3 mechanisms: direct, paradoxical, or iatrogenic. Direct mechanism results from a thrombus originating distal to the pulmonary capillary bed from pulmonary veins, left atrium (especially left atrial appendage in the setting of atrial fibrillation), mitral valve, left ventricle, aortic valve, or sinus of Valsalva. Atrial fibrillation is the most common presumptive culprit in patients with assumed or documented coronary embolism (28%–73%), followed by dilated cardiomyopathy (10%–25%), infectious endocarditis (4%–8%), other valvular heart disease (0%–15%), intracardiac neoplasia (e.g., myxoma; 0%–6%), or marantic (noninfectious) endocarditis in patients with malignancy. Coronary embolism is also attributed to prothrombotic conditions that may promote thrombus/embolus formation within the arterial system, such as systemic autoimmune disease and malignancies
