low-gradient severe aortic stenosis with preserved left ventricular ejection fraction(English)

  • refers to a heterogeneous clinical entity. In severe AS (aortic valve area [AVA] ≤1 cm2) with preserved (≥50%) left ventricular (LV) ejection fraction (EF), low (<40 mm Hg) mean gradients are often interpreted as the consequence of a paradoxical low-flow (LF) state but may also be observed in cases with normal (≥35 mL/m2) stroke volume (SV) index. However, the management of patients with normal flow (NF)/LGSAS-PEF remains elusive and highly debated. It was reported that when the 35 mL/m2 cutoff is used to define LF, almost 40% of normal individuals are in an LF state. Hence, SV index <30 mL/m2 seems a more appropriate cut point in terms of outcome prediction and should be used to define LF in clinical practice
