Monitoring LV remodeling is crucial to assess disease progression in chronic AR. LV end-systolic diameter (LVESD) remains the most commonly used surrogate of adverse remodeling, given its strong association with outcomes and broad availability. However, LV volumes better reflect geometric remodeling and are clinically relevant in chronic AR. AR mechanism can be categorized as follows: Type 1, aortic root dilatation; Type 2, excess cusp motion (prolapse, fenestration); and Type 3, cusp retraction, calcification or endocarditis; or indeterminate mechanism
