• defined as lateral-lateral maximum external thoracic diameter, measured in the standing position and with open arms, by using a rigid ruler coupled to a level, placed at the distal third of the sternum at the level of its maximum depression, as related to echocardiographic measurement of the antero-posterior minimum internal thoracic diameter, on a parasternal long-axis view, as the distance between the point of entry of ultrasound (apex of the sector) and the anterior surface of the vertebral body
